Family Caregivers, a division of Guardian Healthcare, provides services to our clients through our Adult Foster Care and Group Adult Foster Care programs.
Adult Foster Care (AFC) and Group Adult Foster Care (GAFC) are Medicaid-funded programs which provide services to elderly and disabled people who require help with their activities of daily living.
Adult Foster Care (AFC)
Adult Foster Care programs provide support to clients (people aged 16 or older who have a medical condition or disability that impacts daily functioning) and their in-home caregivers. Support includes tax-free stipends for caregivers and free monthly visits from a RN or CM, who will provide training specific to the client’s needs as well as support caregivers in providing the best possible daily care.
Group Adult Foster Care (GAFC)
GAFC programs provide Certified Personal Caregivers to assist with daily living activities to clients who are aged 22 or older, live in subsidized housing or assisted living facilities, and are elderly or disabled. Our Certified Personal Caregivers can provide aid in a client’s home for up to two hours a day, seven days a week.
